RN MDS Coordinator

Fair Haven Retirement Community, located in Birmingham, Alabama, is currently recruiting a registered nurse MDS coordinator to conduct and coordinate the development and completion of Resident Assessment Instruments (RAI) and Minimum Data Sets (MDS) in accordance with current regulations and aligned with Fair Haven’s new “Household” care/service delivery model. The MDS Coordinator works in close collaboration with the household RN Clinical Leader and interdisciplinary household team to accurately assess and deploy individualized, strength-based care plans designed to support the resident’s goals of care and promote resident well-being.

To be an effective RN MDS Coordinator at Fair Haven, you must demonstrate the ability to:

  • Build strong relationships of trust with the resident, family and interdisciplinary team
  • Engage interdisciplinary team members in a common effort to support resident goals of care
  • Structure and effectively lead self-governing teams
  • Manage and analyze data and information such as RAI data, quality measures, hospitalization rates, reimbursement criteria and utilization metrics to evaluate progress, identify opportunities for improvement, exercise mutual accountability and drive quality care and improvement based upon MDS/RAI generated data

Key Qualifications

  • Associate Degree in Nursing (Bachelor’s Degree preferred)
  • Registered nurse (RN) with current Alabama license in good standing
  • RAC-CT certification
  • Minimum of 5 years experience serving as an MDS coordinator in a long-term and short-stay setting
  • Knowledge and understanding of utilization review and Medicare, Medicaid, managed care and other third party payer skilled nursing reimbursement systems
