RN Care Transitions Navigator

Fair Haven Retirement Community, located in Birmingham, Alabama, is currently recruiting a RN Care Transitions Navigator to ensure smooth and safe transitions of care for residents receiving short-term skilled nursing care and rehabilitation services in its new 42-bed rehabilitation center. The RN Care Transitions Navigator serves in a leadership role to achieve desired clinical outcomes for guests during the stay and support a successful discharge back to home and community. As a member of the transitional care team, this RN is responsible for collaborating with Fair Haven partners across all settings to promote utilization of evidence-based best practices and efficient methodologies to promote safe, quality care across the health care continuum.

We are seeking a talented, confident, and experienced registered nurse with care transitions, managed care, and or case management experience who enjoys serving older adults and is motivated by a challenge. Bachelor’s degree in Nursing or currently enrolled in a BSN completion program and two (2) years of related leadership/supervisory experience required or associate degree with at least 10 years leadership/management experience in a long-term care setting. Transitional care, managed care and/or case management, and utilization review experience a plus.

Currently licensed or eligible to be licensed as a Registered Nurse (RN) by the Alabama Board of Nursing is required.
